Name of Form Purpose of Form California Dream Act Application 2023-2024 Available October 1, 2022. This form is used to apply for financial aid for the 2023-2024 academic year by students who qualify for an AB 540 nonresident tuition exemption. Free Application for Federal Student Aid (FAFSA) 2023-24 Available October 1, 2022. This form is used to apply for financial aid for the 2023-24 academic year. Ineligible PLUS Borrower Form 2023-24 Parent(s) listed on the FAFSA can submit this form if they were unable to complete the online PLUS application because of their non-citizen status, a federal loan default, or a grant overpayment. Do not use this form if your credit was denied; see the options if credit is denied section (#9) on the PLUS Application Tips page. You can submit a Parent Contribution Appeal or a Student Contribution Appeal - we will review your appeal, make adjustments (as appropriate) based on your most current financial situation. To request the form, navigate to the Berkeley Financial Aid Forms website, click on the Manage Requests button at the top right, and then click on the + to open the form.

Native American Opportunity Plan - Graduate Students Graduate students - use this form if you need to provide supporting documentation to verify eligibility for Native American Opportunity Plan funding. Native American Opportunity Plan - Undergraduate Students Undergraduate students can confirm eligibility the Native American Opportunity Plan by logging into the Berkeley Financial Aid Forms website.
